All-at-Home Health Care in the Springfield area seeks a Field Physical Therapist for Home Health/Hospice with flexible hours and a per-diem schedule.
The PT assesses patient needs, develops treatment plans with physicians, delivers active and passive therapies, and educates patients and caregivers on daily living activities.
Required is current PT license, 1 year home care experience, valid driver’s license, and reliable transportation; travel to 3–5 visits weekly is expected.
