NNCC is seeking Care Coordinator for our funded New Life – Nueva Vida Program
. These are in-person positions and will provide services in the greater Springfield area (Holyoke, Chicopee, West Springfield, Westfield, etc.).
Position Summary
:
The Care Coordinator position provides case coordination and other essential program services to men of color (African American/Black & Hispanic/Latino), who are 18 & older, and re-entering from incarceration in the Greater Springfield area. The Care Coordinator will be responsible for leading with trauma informed care, supporting the multiple pathways of recovery, supporting family reunification, incorporating harm reduction strategies, as well as, providing support and access to training, employment, housing, and other social skills in order to facilitate a successful re-integration into communities throughout Western Massachusetts.
All of the care coordination services and supports are to be provided preferably by BIPOC men with lived and living experiences of incarceration, SUD and/or Mental Health Recovery, and other related conditions.

PAY – Non-Exempt: $50,000 – $52,000 annually.
