Baystate Health is a not‑for‑profit, integrated healthcare system in western Massachusetts that includes an academic medical center, three community hospitals and the region’s largest multispecialty group. The flagship hospital, Baystate Medical Center, hosts the University of Massachusetts Chan Medical School‑Baystate. We are seeking an experienced Advanced Practitioner to join our Palliative Care division.
Position Highlights- Join a vibrant Palliative Care team that includes 6 Advanced Practice Providers (APPs), 6 physicians, 2 social workers, spiritual support and a highly supportive clinical staff dedicated to evidence‑based, patient‑centered care.
- Work in a community palliative care program focused on providing care in a supportive environment to patients and families facing advanced illness and end‑of‑life challenges.
- Collaborate closely with an interdisciplinary team consisting of Social Work, Nursing, Spiritual Care Services, Nurse Practitioners and Physicians. This includes holding family meetings, discussing goals of care with patients and families, and developing care plans.
- Assess and manage symptoms of patients with advanced illnesses in a consultative model with primary care providers and other specialists. Collaborate with the inpatient palliative care team while assisting with the transition to hospice when appropriate.
- Participate in interdisciplinary team meetings that foster collaboration, professional development and continuous growth.
- Utilize an established telehealth platform that allows clinicians to deliver care remotely.
- This is an .8 FTE community‑based position. The schedule offers flexibility and a great work/life balance. Mileage reimbursement and a laptop are provided. Regions are assigned based on geographic location.
BC Nurse Practitioner or Physician Assistant with clinical experience in home care, palliative care or geriatrics settings.
CompensationPalliative Care AP: $129,780 - $164,400 for 1.0 FTE, $103,824 – 131,520 for .8 FTE.
