. Thisis an exciting opportunity to makea meaningful impactby coordinating carefor patients with chronic and complex conditions while partnering closely with physicians, advanced practice providers, and amultidisciplinary careteam.
As an Ambulatory RN, you will provide clinical triage, patient assessment, education, care coordination, and ongoing follow-up. You'll manage patient portal communications,conduct nurse visits, support treatment plans, and serveas a trusted resource for patients throughout their carejourney. The ideal candidate is an excellent communicator who thrives ina collaborative, patient-centered environment and is committed to delivering exceptional ambulatory care.
SCHEDULE:- Full-Time
- Monday–Friday |8:00 AM–4:30 PM
- Current Massachusetts Registered Nurse (RN) license.
- Current AHA BLS certification.
- Minimum of two(2) years ofRN experience.
- Strong assessment, triage, patient education, andcare coordination skills.
- Excellent verbal, written, and interpersonal communication skills.
- Demonstrated ability to collaborate effectively with providers and interdisciplinary teams.
- Commitment to delivering compassionate, patient-centered care to individuals from diverse backgrounds.
- Ambulatory or specialty practice nursing experience.
- Experience caring for patients with chronicor complex medical conditions.
- Adult and pediatric nursing experience, ora willingness towork with pediatric patients.

Typically, a new hire can expect a starting salary somewhere in the lower part of the range. Actual salaries may vary by position and will be determined based on the candidate's relevant experience. No employee will be paid below the minimum of the range.
Pay ranges are listed as hourly for non-exempt employees and based on assumed full time commitment for exempt employees.
