** Registered Nurse to join our Rheumatology team**. This is an exciting opportunity to make a meaningful impact by coordinating care for patients with chronic and complex conditions while partnering closely with physicians, advanced practice providers, and a multidisciplinary care team.

As an Ambulatory RN, you will provide clinical triage, patient assessment, education, care coordination, and ongoing follow-up. You'll manage patient portal communications, conduct nurse visits, support treatment plans, and serve as a trusted resource for patients throughout their care journey. The ideal candidate is an excellent communicator who thrives in a collaborative, patient-centered environment and is committed to delivering exceptional ambulatory care.##

* Monday–Friday | 8:00 AM–4:30 PM## ##

* QUALIFICATIONS:

*** Current Massachusetts Registered Nurse (RN) license.
* Current AHA BLS certification.
* Minimum of two (2) years of RN experience.
* Strong assessment, triage, patient education, and care coordination skills.
* Excellent verbal, written, and interpersonal communication skills.
* Demonstrated ability to collaborate effectively with providers and interdisciplinary teams.
* Commitment to delivering compassionate, patient-centered care to individuals from diverse backgrounds.
* Ambulatory or specialty practice nursing experience.
* Experience caring for patients with chronic or complex medical conditions.
* Adult and pediatric nursing experience, or a willingness to work with pediatric patients.#
