- Verify the Plan of Care prior to each visit
- Provide care according to RN delegation, physician orders, assessment data, and established standards and guidelines
- Incorporate patient care goals into nursing visits and document them in skilled nursing notes
- Communicate significant findings, problems, and changes in patient condition or environment to the RN Case Manager, physician, clinical supervisor, and other involved personnel
- Report unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician
- Implement patient safety plans using patient, family, and community resources
- Teach patients appropriate self-care techniques while delivering quality care
- Suggest revisions to the Plan of Care and notify the RN Case Manager of identified care issues
- Provide substantial and specialized nursing services under the direction of a Registered Nurse or physician
- Collaborate with the RN Case Manager during normal business hours
- Document medication regimens and update medication profiles
- Treat patients and caregivers in the highest and most effective manner
- Implement actions to prevent avoidable hospitalizations and emergency room visits
- Provide positive, supportive communication to physicians, patients, families, visitors, and agency personnel
- Report to the Clinical Team Manager
- High school diploma or GED
- Graduate of an accredited certificate, diploma, or associate school of nursing
- Current state license as a Licensed Vocational/Practical Nurse
- 1 year of experience in a clinical care setting; hospice experience preferred
- Home health environment experience preferred
- 50% travel required
- Ability to sit, stand, bend, lift, and move intermittently
- Ability to lift 50–100 lbs.
- Valid driver’s license and insurance
- Reliable transportation to perform job tasks
Demonstrates expertise in patient care delivery, including the ability to implement care plans, communicate effectively with healthcare teams, and educate patients on self-care techniques. Proficient in documenting care and medication regimens while ensuring patient safety and compliance with established standards.
