Hospice of Western & Central Massachusetts is seeking a compassionate and experienced Hospice RN Case Manager to join our team! As a key member of our care team, you will provide exceptional hospice care to patients and their families, building relationships and delivering comprehensive services that support their unique needs.
Shift is Monday through Friday 8a-4:30p. This is an on-the-road position with mileage reimbursement. Up to $49 an hour with weekly pay.
Responsibilities- Plan, organize, and direct hospice care for patients and families in their homes and communities
- Conduct initial, comprehensive, and ongoing assessments to determine patient and family needs
- Develop and implement patient care plans, collaborating with patients, families, and healthcare providers
- Provide professional nursing care, utilizing elements of the nursing process
- Supervise hospice aides and participate in revising patient care plans as necessary
- Collaborate with healthcare providers to ensure patient continuity of care
- Provide education and guidance to patients and families on disease management, wound care, and symptom management
- Participate in patient care conferences to review patient progress and make adjustments to care plans
- 1-2 years of recent acute care experience in an institutional setting and 1-2 years of recent experience in home care
- CPR certification
- Active and unencumbered RN license in Massachusetts
- Bachelor's degree in Nursing with 1 year of hospice care experience preferred
