Position: Shared Living Placement Specialist

Latham Centers is seeking a Shared Living Placement Specialist to support our Adult Services team. This role coordinates person-centered placements for individuals receiving services through the Massachusetts Department of Developmental Services (DDS), while supporting provider recruitment, respite coordination, and placement stability. The ideal candidate is organized, relationship-driven, and passionate about supporting adults with complex needs in community-based settings.

Schedule

Monday 9:00am-3:00pm, Tuesday-Thursday 9:00am-6:00pm, Friday 9:00am-3:00pm

Location

Yarmouth, MA (Position will be based out of Yarmouth, but also covering Cape as well as southeastern MA. Must use own vehicle for transportation, mileage reimbursed)

Compensation

$24.00-$25.00 per hour

Key Responsibilities
  • Coordinate person-centered placements for individuals served through DDS
  • Assess individual needs, strengths, preferences, and support requirements
  • Manage referrals, intake documentation, and placement records
  • Coordinate interviews, meet-and-greets, home visits, and transitions
  • Recruit and develop Shared Living providers through outreach and networking
  • Maintain provider pipelines and placement tracking systems
  • Coordinate respite services and maintain resource lists
  • Monitor placement progress and address barriers to stability
  • Support crisis intervention and problem-solving related to placements
  • Collaborate with case managers, clinicians, nursing, program leadership, and DDS
  • Maintain accurate documentation in compliance with agency and DDS requirements
  • Support administrative processes including CORI, Mass Health, DTA, and Social Security documentation
Requirements
  • High School Diploma or GED required
  • Bachelor’s degree in a related field preferred
  • Valid driver’s license
  • Ability to physically intervene in instances of extreme behavior
  • Basic computer skills
  • Independently motivated, innovative, and highly organized
  • Experience working with adults with special needs preferred
  • Administrative or coordination experience preferred
  • CPI, CPR/First Aid, and MAP certification (or ability to obtain)
