Job Overview
NOT Remote
Responsible for assisting older at-risk adults within the Protective Services department, and Anchor program and has the ability to receive screened‑out referrals pertaining to housing and provide case assistance to the Protective Services Workers with collateral calls in order to appropriately determine/ assess risk.
Responsibilities- Appropriately use documentation systems such as APS and Form Site to document all contact with collaterals, intake assessments, and enter case progress notes.
- Receive referrals pertaining to housing and request case assistance to minimize backlog and assess risk in a timely manner.
- Complete collateral calls for active investigations and housing intakes within 48 hours to allow time for review of APS and Form Site records.
- Adhere to supervision requirements per GSSSI Policy, request additional guidance for complex situations, and notify the overseeing PSS of completion of collateral calls to ensure appropriate case assignment.
- Track case assistance provided for each assigned case in an Excel spreadsheet, including completion dates of collateral calls; debrief direct PSS so they can debrief the Interdisciplinary team. Provide a monthly Excel spreadsheet to the overseeing Protective Services Supervisor and Director.
Requires English/Spanish fluency.
Work Hours35 hours/week.
