Position SummaryThe HOPWA Case Manager provides comprehensive housing-focused case management and supportive services to individuals and families living with HIV/AIDS who are eligible for assistance through the Housing Opportunities for Persons with AIDS (HOPWA) Program. The Case Manager works collaboratively with clients to develop housing stability plans, connect them to healthcare and community resources, and assist them in achieving long‑term self‑sufficiency and improved quality of life.
This position ensures compliance with U.S. Department of Housing and Urban Development (HUD) HOPWA regulations, agency policies, confidentiality requirements, and funding guidelines while promoting housing stability and positive client outcomes.

Client Services & Case Management- Conduct intake assessments and determine program eligibility in accordance with HOPWA program guidelines.
- Develop individualized housing stability plans and service plans with clients.
- Maintain an active caseload and provide ongoing case management services.
- Conduct comprehensive assessments to identify client strengths, needs, barriers, and goals.
- Provide crisis intervention, advocacy, supportive counseling, and referrals.
- Conduct home visits and regular client follow‑up as required by program standards.
- Monitor client progress and update service plans as necessary.
- Assist clients in obtaining and maintaining safe, affordable, and stable housing.
- Support clients in accessing healthcare, behavioral health services, substance use treatment, transportation, employment services, and other community resources.
- Assist clients with applications for benefits including SSI/SSDI, SNAP, Mass Health/Medicaid, and other public assistance programs.
- Assist clients with housing searches, landlord engagement, lease applications, and housing placement activities.
- Coordinate HOPWA‑funded housing assistance programs, including:
- Tenant‑Based Rental Assistance (TBRA)
- Short‑Term Rent, Mortgage, and Utility Assistance (STRMU)
- Permanent Housing Placement Services
- Conduct housing inspections and document Housing Quality Standards (HQS) compliance as required.
- Support housing retention efforts through landlord mediation, tenant education, and eviction prevention strategies.
- Monitor housing stability and address barriers that may place housing at risk.
- Maintain accurate, timely, and complete client records and case files.
- Complete assessments, service plans, progress notes, and required program documentation.
- Ensure compliance with HUD HOPWA regulations, agency policies, grant requirements, and contractual obligations.
- Enter and maintain client information within HMIS and other designated data management systems.
- Participate in file reviews, audits, monitoring visits, and quality assurance activities.
- Prepare reports and documentation required by funders and agency leadership.
- Protect client confidentiality in accordance with HIPAA, HUD regulations, and agency standards.
- Develop and maintain effective partnerships with housing providers, healthcare organizations, HIV/AIDS service agencies, and community partners.
