Clinical Case Manager/Educational Liaison – Full Time

Full‑time Clinical Case Manager/Educational Liaison (CCM) in Residential Services focuses on clinical and collateral case management, ongoing assessments, and aftercare. The CCM also serves as the Educational Liaison, supporting youths’ educational plans and ensuring IEPs meet needs in the school environment. The candidate advocates for youth in educational settings, attends school meetings, and maintains relationships with students during school, after school, and after‑hours on‑call support.

• Provide clinical and collateral case management, conduct ongoing assessments, and manage aftercare for youths in residential services.
• Act as Educational Liaison: support youths’ learning, ensure IEPs are comprehensive and meet school needs.
• Advocate for youth in the educational setting, attend school meetings, and maintain relationships with students.
• Be available during school hours, after school hours, and after‑hours on‑call to address emerging needs.

• Master’s Degree
• Completion of required trainings after hiring, including but not limited to CPI, CPR, First Aid
• Approved CORI & DCF background checks (performed by the EEC)
