PT Navigator Bridges; Medically Complex Program

Location: MCS – SOS Office 505 East Columbus Ave., Springfield, MA
Hours: Part-Time 20 hrs/week
Program: Family Support

• Service Navigators in the Medically Complex Program support families who have a family member with an Intellectual Disability and/or complex medical challenges and needs.
• Support families through the eligibility process for DDS services.
• Provide information or referral services to assist families with other areas of need.
• Case management, accessing generic services, assessing needs, developing Family Support Plans.
• Supervise Direct Support Professionals (if in DESE program).
• Maintain quality of programming through adherence to agency standards and DDS regulations.
• Client navigation for the individual/family.
• When applicable, hiring and supervising direct care staff.
• Work closely with DDS Service Coordinators.
• Outreach to generic services and collaborate with other state agencies and service providers.
• Maintain accurate financial records.
• Ensure timely completion of all documentation and other duties as required.
• Preference for individuals who are family members of a child/adult with complex medical challenges.

• Valid driver’s license and dependable vehicle.
• BS or BA in a related field with one year of experience preferred.
• At least high school diploma with 3 years of experience in Human Services.
• Bilingual (English/Spanish) is a plus.
