Job DescriptionSchool Based Services (SBS), part of the Positive Regard Network, invites applicants to apply for an anticipated opening for a Clinical Case Manager (CCM) position to provide intensive counseling and support to students in a school setting through group therapy, case management between school and home/ancillary services, and support to the classroom team. This anticipated position would be scheduled 30 hours per week, Monday-Friday, generally working August through June during the school year, as a 10 month position, working in a public school setting.
The position requires a Master’s degree in Social Work, Psychology, or related field; MA license in a clinical field (LMHC, LCSW, LICSW) required (or license eligible with a professional development plan for licensure within one year), along with considerable experience in the field of counseling (2+ years in a diverse clinical setting).
Position Responsibilities- Provides direct clinical service, e.g., individual and/or group therapy as outlined in Individual Education Plans (IEPs) and treatment plans of students.
- Primary contact for parents/guardians. Balance a professional relationship with the understanding that they are the primary support for the student’s school-to-home relationship. Provides family outreach as needed.
- Makes and receives all case calls with parents/guardians and other tertiary providers. Oversees all case call documentation of assigned students. This includes daily attendance and completing home visits when students are not present in school for a number of days in a row (usually accompanied by another staff member as well).
- Develops individual treatment plans/behavioral plans for each student on caseload and establishes clinical treatment goals and objectives to be included in overall service plan for each student. Attends in-school meetings with all team members to create plans to assist students to be successful each day.
- Provides emergency clinical services including referrals based on psychiatric crises.
- Liaisons with outside agencies and authorities, including social services organizations and school district personnel, and represents the school professionally at community meetings and events, including court, conferences, etc., as needed.
- Oversees that intake packets are completed once a year for the students on their caseload.
- Arranges appointments for students as appropriate and needed. This may include accompanying students to school psychiatry appointments.
- Writes clinical summaries, case notes, IEP goals/benchmarks, progress reports, discharge summaries/transition plans and other clinical reports as requested or as needed. This can also include preparing clinical updates for presentations at district team meetings and attending all team meetings.
- Facilitates ongoing communication and attends morning and afternoon staff meetings and/or treatment/behavior planning meetings as needed and available. Consults with and maintains ongoing communication with Site Administrator, Educational Administrator, Teachers, Floor Manager, and/or Lead Interventionists, as appropriate. Informs staff of outside issues in a timely manner.
- Participates in scheduled supervision, training and staff meetings. Plans and provides training for school staff as needed or requested.
- Adheres to professional license standards while upholding the school model of treatment.
- Maintains professional licensure and participates in conferences, workshops or other courses for professional development.
- Supervises college interns as assigned.
- Initiates, organizes and manages 51A paperwork when necessary and in a timely manner, upholding confidentiality in its highest regard. All consideration of filing of 51A’s must be communicated to and consulted with the Director prior to filing.
