LocationWestfield, MA (serving Westfield, Springfield, and surrounding areas)
ScheduleMonday–Friday, 8:00 AM–4:00 PM or 9:00 AM–5:00 PM
About the RoleArbor Associates is seeking a dedicated Case Manager to support individuals with developmental disabilities and behavioral health needs across the Westfield, Springfield, and surrounding communities. This is a full-time temporary position covering a leave of absence, with a consistent Monday–Friday daytime schedule. This role is hands‑on and community‑based — you’ll work directly with clients in their homes and in the community to build independence, stability, and quality of life.
WhatYou’ll Do
- Support clients in developing and maintaining daily living skills
- Assist with housing stability and household management
- Coordinate and reinforce medical appointments and medication routines
- Provide budgeting support and financial literacy coaching
- Facilitate community integration and social skill development
- Transport clients to appointments and community activities using your personal vehicle
- Document progress and maintain case records in accordance with program requirements
