We are looking to hire a Transitional Assistance Coordinator to join our Guardian Angel Senior Services team for our Central MA Locations.
Job DescriptionThe Transitional Assistant Coordinators will assist Guardian Angel in services for adults with developmental disabilities, individuals who are blind or visually impaired, and adolescents with brain injury.
This position must be motivated by being an instrumental part of giving a participant at a facility level of care the opportunity to rejoin community living with support in place to live a safe and fulfilling life.
Work collaboratively and under the instructions of discharge planners and State Case Managers.
Assist in the execution of service plans designed to develop, maintain, and/or maximize the participant's independent functioning in self-care, physical, and emotional growth, socialization, communication, and vocational skills.
Responsibilities- Assist with participant transition from skilled nursing facilities to community living with appropriate support.
- Assist with housing search, voucher and housing applications.
- Attend housing appointments with participant for housing authorities, such as an Social Security Card.
- Set up apartments as preferred by participant.
- Maintain active communication with MRC case manager with updates on participant.
- Attend transition meetings prior to participant discharge.
- Track and document direct services, housing application, shopping expenses and all communication related to participants.
- Initiate home assessments on housing space for suitable housing goods and furnishings.
- Purchase furniture, housing goods and all essential items based on participant preferences.
- Set up utilities services as needed.
- Adequately update services activity log, delivery reports and submit reimbursement in a timely fashion.
- Experience providing community-based services to individuals with disabilities is a plus.
- Valid driver’s license, clean driving record, and a reliable, clean, insured vehicle; willing to travel (mileage reimbursed).
- Maintain confidentiality of sensitive information.
- Utilize effective time management techniques to ensure tasks are prioritized and deadlines met.
- Comply with State/Federal regulatory acts and agency policies.
- Respect and maintain client’s confidentiality.
- Communicate effectively in the language and communication style of the participants and their families.
- Experience with housing preferred.
