Signify Health, part of CVS Health, is seeking a Per Diem Physician to provide In-Home Health Evaluations. You will engage with patients in the comfort of their own homes to bridge gaps in care and support health plan objectives.
The visit lasts about 45 minutes, including medication and medical history review, a physical evaluation, and, if ordered by the health plan, additional tests. You may travel locally within licensed states and perform virtual visits as needed.
