* Key Responsibilities:

* · Treat all clients, referral sources, stakeholders, and team members with dignity and respect.
· Develop and maintain a working knowledge of disabilities and potential assistive technology/accommodations.
· Compile and review vocational, medical, psychological, and educational information to assist clients with skill development.
· Contribute to and/or develop individualized service plans with clients.
· Conduct activities identified within clients’ individualized service plans.
· Coordinate paid, work-based learning experiences, including orientation and paperwork.
· Transport clients to meet goals identified in their service plans.
· Monitor and report on progress toward individualized service plan goals.
· Ensure authorizations for services are current.
· Instruct and facilitate clients’ skill development in vocational, social, and independent living contexts.
· Develop strategies and curriculum for skill development based on individualized service plans.
· Establish community-based sites for skill development and career planning.
· Provide onsite job supports to assist clients with their job duties.
· Educate employers regarding the benefits of hiring persons with disabilities.
· Serve as a liaison between employers, clients, and co-workers.
** Education, Experience, and/or Credential
Qualifications:

* · A high school diploma or equivalent certificate is required; an associate's or bachelor's degree in an applicable field is preferred.
· Experience in rehabilitation, employment services, case management, social services, or related fields is preferred.

* Additional Qualifications:

* · Successful completion of background checks.
· Must have current driver's license with an acceptable driving record and auto insurance.
