SERC Physical Therapy, a brand partner of Upstream Rehabilitation, is looking for a Patient Care Coordinator to join our team in Midtown KC, MO.

This entry‑level office role is responsible for maintaining a pleasant and consistent daily operation of the clinic while providing excellent customer‑service skills.
A Day in the Life- Greets every person who enters the clinic in a friendly and welcoming manner.
- Schedules new referrals received by fax or by telephone from patients and physician offices.
- Verifies insurance coverage for patients.
- Collects patient payments.
- Maintains an orderly and organized front office workspace.
- Other duties as assigned.

The Patient Care Coordinator (PCC‑I) supports clinic growth through excellence in execution of the practice management role and patient intake processes. The PCC‑I works in collaboration with the Clinic Director to carry out efficient clinic procedures and supports the mission, vision, and values of Upstream Rehabilitation.
Responsibilities- Collect all money due at the time of service.
- Convert referrals into evaluations.
- Schedule patient visits.
- Provide customer service.
- Create an inviting clinic atmosphere and make welcome calls to monitor and influence arrival rates.
- Manage schedule efficiently, document routing, personal overtime, and non‑clinical documentation.
- Manage deposits, caseload, discharge candidates, progress notes, and insurance reporting.
- Monitor clinic inventory.
- Attend required training with Territory Field Trainers and complete quarterly compliance training.
- High School Diploma or equivalent.
- Excellent communication skills sufficient to relate well with Business Office and Field leadership.
- Ability to multitask, maintain organizational detail, meet deadlines, and work with little to no supervision.
- Efficient time‑management and presentation skills as a member of a team.
- Work inside an environment with protection from weather but exposure to normal indoor temperature variations and noise.
- Full‑time position within normal business hours Monday through Friday; occasional evenings and weekends or additional hours may be required.
- Primarily sedentary work with intermittent standing.
- Frequent communication requiring clear hearing and speaking, and frequent computer use.
- Repetitive typing motions and sustained near‑acuity visual focus.
- Occasional walking, standing, pushing or pulling up to 20 lbs.; rarely crouching, kneeling, stooping, climbing stairs, or reaching above shoulder height.
