Looking to make a difference in parents and newborns lives located in Springfield and Greene County?
The Community Health Advocate vacancy is located within the Family Connects program a part of the Springfield-Greene County Health Department. The Family Connects program supports parents in caring for their newborn by offering a nurse home visit at no cost for all families living in Greene County. The Community Health Advocate position will establish initial contact with families at the hospitals and engage, recruit, and schedule families for nurse home visits.
The position will work to build and maintain relationships with hospital staff and other referring agencies. This position will also conduct post-visit connection phone calls with families, track services contacted and received and provide referrals when necessary. The Advocate will work as an active member of the Family Connects team by attending all regular staff meetings, in-service trainings and other meetings as required.

Experience, Education, and Training- At least two years of college or vocational education in a Public Health Discipline or related field;
AND
- One year of related work experience;
- Directly related work experience can be substituted for the education requirement on a year-for-year basis.
- If operating a motor vehicle during the course of performing job duties, must possess a valid driver's license, and provide required liability coverage.
