PRIMARY PURPOSE OF JOB

This is a professional full‑time, hourly Targeted Case Management (TCM) position within the Missouri Department of Mental Health (DMH) Developmental Disabilities (DD) system. Support Coordinators assist individuals with developmental disabilities in gaining access to needed medical, social, educational, and other services, as defined in the Missouri Division of Developmental Disabilities Targeted Case Management program and in accordance with federal Medicaid guidelines.

ESSENTIAL FUNCTIONS AND PERFORMANCE AREAS
  • Manage a diverse caseload of moderate size and complexity.
  • Interview individuals, their families, and other responsible parties. Assist in completing necessary documents. Collect basic data and obtain appropriate additional information from other agencies. Participate in the interdisciplinary team meetings to review each case.
  • Prepare and complete case notes, quarterly reviews, annual reviews, support monitoring and any other required reports concerning the Person‑Centered Support Plan (PCSP) in accordance with Department of Mental Health (DMH) guidelines, Medicaid regulations and Abilities First policy.
  • Complete all required documentation (PCSPs, waiver paperwork, support monitoring, etc.) in a timely manner following DMH regulations, Medicaid guidelines, Abilities First policy, and all applicable regulatory requirements.
  • Complete and enter casenote documentation into SETWorks billing system daily, no later than 72 business hours after TCM activity.
  • Maintain a monthly/yearly Service Goal of billable 75% of hours worked, ensuring service provided is documented in the SETWorks system.
  • Maintain individual’s client record in accordance with DMH and Medicaid guidelines.
  • Facilitate the interdisciplinary team’s development of the PCSP for everyone served.
  • Attend interdisciplinary team meetings to discuss individuals’ eligibility, PCSP, progress, and possible changes in supports or services.
  • Prepare Utilization Review justification and service authorizations and arrange for individuals to access services.
  • Monitor services to ensure that the terms of the authorization are being fulfilled by the contracted providers, to check on quality of services, and to review individual progress according to the state support‑monitoring directive. If any concerns are noted complete proper follow up following the Service Monitoring Directive. If the concern remains unresolved complete follow up notifying the Senior/Assistant Director and Director of any concerns needing addressed.
  • Develop individualized goals and outcomes with each individual recognizing and identifying changing his/her wants and needs. Identify any available natural supports. Provide information about appropriate community resources.
  • Continuously assess the supports in place for the individual and ensure they are effective toward supporting the individual to reach his/her goals as identified in the PCSP.
  • Collaborate with Senior Support Coordinator to obtain funding for needed services that cannot be met using natural supports.
  • Maintain contact with every individual on caseload as per DMH guidelines. This includes monthly face‑to‑face visits and completion of monthly Support Monitoring according to DMH guidelines with all individuals receiving Residential services. Document and report any concerns following the Service Monitoring directive.
  • Meet face‑to‑face with individuals on caseloads that Abilities First serves. Verify documentation of PCSP and services with the contracted provider.
  • Review provider documentation including daily progress notes, provider summaries, incident reports, and service authorizations that result in provider billing for accuracy of services authorized.
  • Visit service agencies, attend meetings, and confer with senior staff to become informed concerning community resources for persons with developmental disabilities.
