The Clinical Pharmacist is responsible for providing comprehensive medication management services to PACE participants through collaboration with the interdisciplinary team (IDT). This position supports the PACE philosophy of promoting participant independence, health, safety, and quality of life through evidence-based medication therapy management, medication safety initiatives, chronic disease management, and regulatory compliance.
The Clinical Pharmacist serves as the organization's medication expert, ensuring optimal pharmacologic outcomes for older adults with complex medical conditions, multiple chronic diseases, and polypharmacy. The Clinical Pharmacist evaluates medication regimens, identifies and resolves medication-related problems, educates participants and caregivers, and collaborates with providers to promote safe, effective, and cost-conscious medication use.
Duties:
- Conduct comprehensive medication reviews (CMR) for PACE participants to identify and resolve drug therapy problems.
- Document and communicate recommendations to the healthcare team and follow up as needed.
- Develop and recommend individualized pharmaceutical care plans based on participant-specific goals and clinical needs.
- Provide evidence-based recommendations to physicians, nurse practitioners, physician assistants and other members of the interdisciplinary team.
- Facilitate prescription management to appropriate pharmacy based on insurance payer and patient specific needs.
- Support tapering efforts and education regarding polypharmacy management in complex cases.
- Manage billing processes for services provided, ensuring compliance with payer requirements and securing appropriate reimbursement
- Provide individualized patient education and counseling focused on medication use, side effects, adherence strategies, and safety. Monitor for therapeutic outcomes and make recommendations based on participant response and changing clinical conditions.
- Address stigma, misconceptions, and concerns related to psychiatric, pain, and substance use disorder medications.
- Collaborative care & clinical integration:
- Act as a clinical resource for PACE providers related to medication management.
- Engages with pharmacy staff as part of an interdisciplinary team to support comprehensive, patient-centered care
- Contribute medication-related recommendations during participant care conferences.
- Support transitions of care by performing medication reconciliation following hospital admissions, discharges, emergency department visits, and specialist consultations.
- Assist the care team in identifying medication-related barriers to treatment adherence and successful outcomes.
- Ensure compliance with state and federal law, standards of care, and support regulatory readiness and compliance activities related to pharmacy services.
- Lead or participate in medication safety initiatives and quality improvement programs.
- Monitor and report adverse drug events, medication errors, and near misses.
- Recommend strategies to reduce high-risk medication use and improve medication safety.
- Evaluate medication utilization and prescribing patterns to support quality care and cost-effective treatment.
- Attains all agreed to goals and objectives within specified time frames, as part of the Clinic's overall mission.
- Responsibly follows the Clinic Exposure Control Plans/Bloodborne and Airborne Pathogens.
- Demonstrates respect and regard for the dignity of all patients, families, visitors and fellow employees to ensure a professional, responsible and courteous environment.
- Promotes the education and development of students, interns, residents, apprentices, and other new staff by sharing expertise, responding to questions, and fostering a positive and supportive learning environment.
