Traveling Home Health Physical Therapist
Traveling home health physical therapist for a 13-week contract in Springfield, Missouri. Provides comprehensive patient evaluations, develops individualized treatment plans, and manages documentation and care coordination. Earns up to $2,700 per week based on experience.
Key Responsibilities:- Conduct comprehensive patient evaluations in the home health setting
- Develop individualized treatment plans focused on improving mobility, strength, balance, and independence
- Perform start-of-care assessments, recertifications, and routine home health visits
- Complete accurate OASIS documentation and maintain compliance with home health regulations
- Serve as a case manager coordinating care with physicians, nurses, therapists, and interdisciplinary teams
- Educate patients and caregivers on therapeutic exercises, mobility strategies, and safety precautions
- Monitor patient progress and modify treatment plans as needed
- Accurately document all patient interactions and treatment outcomes using Matrix EMR
- Deliver compassionate, patient-centered care while promoting functional independence and quality of life
- Doctorate or Master's degree in Physical Therapy from an accredited program
- Active Missouri Physical Therapist (PT) license required
- Strong experience with OASIS documentation, case management, start of care assessments, and recertifications required
- Excellent communication, organizational, and time management skills
- Ability to work independently in a home health environment
Skills:
- Current CPR/BLS certification
- Home health experience highly preferred for travel assignments
- Experience with Matrix EMR
