Community Health Worker – Pediatrics

Baystate Health, a nationally recognized leader in healthcare quality and safety, is seeking a full‑time Community Health Worker (CHW) for its Springfield, MA location, including home visits as needed.

Compensation
  • Minimum: $21.52
  • Midpoint: $24.73
  • Maximum: $29.24
Schedule
  • Full‑time, 40 hours per week
  • Monday – Friday

Responsibilities
  • Develop drive and determination to learn and grow in the CHW role.
  • Engage positively with patients, families and the medical team.
  • Target and outreach to specific patient populations, including home and community visits.
  • Act as key point person between patients and providers of healthcare and social services.
  • Work effectively with people from culturally diverse backgrounds, including translators, to reduce barriers.
  • Assist with medication adherence, appointment reminders, transportation and other support services.
  • Educate patients on all aspects of the healthcare delivery system and assist with enrollment of new members.
  • Identify and utilize community resources via an approved database and demonstrate use of EMR systems.
  • Schedule and accompany patients to appointments and assist with application completion for eligible programs.
  • Communicate between providers and the clinical team; collaborate within a multidisciplinary team.
  • Build and maintain positive relationships with clients, providers, supervisors and staff.
  • Continuously expand knowledge of community resources, public health prevention and evidence‑based programs.
  • Document all activities within the EMR and community service plans, ensuring clear data collection.
  • Complete Care Needs Screenings and other applicable assessments to provide appropriate referrals.
  • Coach patients in managing chronic conditions and self‑care.
  • Implement culturally and linguistically tailored educational and support activities.
  • Collaborate with the care team to track, monitor and report disease‑related and patient tracking measures.
Qualifications
  • High School Diploma or GED (required).
  • Enrolled or willing to enroll in the CHW certification program.
  • CHW certification (preferred).
  • Some exposure to working in a community setting (preferred).
  • Driver’s license and reliable vehicle for daily home visits (required).
Skills & Competencies
  • Excellent interpersonal, communication and writing skills.
  • Strong time management and organizational abilities.
  • Proficiency with basic computer skills, Microsoft Office and internet research.
  • Knowledge of confidentiality and handling sensitive information.
  • Willingness to work some weekend or evening hours as needed.
  • Commitment to professionalism and respect for all human beings.
Education

GED or HiSET (required).

Certifications
  • Certified Community Health Worker – State of Massachusetts.
  • Driver’s License, Massachusetts (required).
