Field Care Manager, Behavioral Health

• Manage a caseload and complete comprehensive assessments in members’ homes, community-based settings, and telephonically
• Provide clinical support and guidance in collaboration with Interdisciplinary Care Team participants
• Develop and coordinate individualized care plans
• Identify and address barriers to care while advocating for optimal member outcomes
• Review, assess, and complete medical attestations and provide ongoing clinical oversight
• Ensure services are delivered in the least restrictive setting through ongoing assessment of care needs
• Develop and modify Individual Care Plans with applicable care team members
• Help members and caregivers access social, housing, educational, and community resources
• Serve as the primary point of contact for the Interdisciplinary Care Team
• Coordinate with members, care team participants, and external resources for comprehensive care delivery
• Travel within Wayne and Macomb Counties for field-based, face-to-face assessments
• Work Monday through Friday during specified Eastern Time schedules, with overtime flexibility as needed

• Must reside in the State of Michigan
• Ability to travel to Wayne and Macomb counties (75%)
• Bachelor’s degree in Social Work (BSW) from an accredited institution
• Active Michigan social work licensure in good standing: LLBSW, LBSW, LLMSW, LMSW, or LCSW
• Limited license holders must demonstrate intent and active progress toward full licensure
• 2+ years of post-degree clinical or care management experience in a behavioral health, medical, or community-based setting
• Experience working with adult populations, including chronic conditions and disease management
• Knowledge of community health resources, social service agencies, and support programs
• Ability to conduct face-to-face assessments with Medicare/Medicaid members and engage telephonically
• Valid driver’s license with reliable transportation
• Strong communication and interpersonal skills
• Ability to work with minimal supervision and independently within defined role scope
• Proficiency using electronic medical records and healthcare-related software systems
• Excellent keyboard and web navigation skills
• Intermediate to Advanced computer skills
• Experience with Microsoft Word, Outlook, and Excel
• Patient-facing role requiring TB screening
• Personal vehicle liability insurance meeting applicable requirements

Proficient in utilizing community health resources and electronic medical records to support patient care and engagement.
