Position: RN (Registered Nurse) Hospice Case Manager

Overview

, our RN Case Managers walk alongside patients and families during one of life's most vulnerable seasons. You'll provide skilled, compassionate care while helping families navigate difficult decisions with clarity, dignity, and heart. Every visit matters. Every conversation counts. You won't do this work alone. You'll be supported by a strong interdisciplinary hospice team that believes in teamwork, purpose, and caring for the caregiver just as much as the patient.

• Salary Range: $75,000 - $80,000
• Full-Time & PRN Opportunities
• Sign-On: $5,000

What You'll Do
  • Provide compassionate hospice nursing care to patients and families
  • Educate patients and caregivers on the hospice process and plan of care
  • Guide families through goals-of-care discussions and healthcare decisions
  • Conduct initial and ongoing comprehensive assessments
  • Perform hospice evaluations and informational visits
  • Collaborate with physicians, Hospice Medical Directors, and interdisciplinary team members
  • Coordinate care to ensure patient and family needs are fully met
  • Communicate changes in patient condition and update plans of care as needed
What You'll Need
  • Active Missouri or Compact RN license
  • Hospice or Long-Term Care nursing experience preferred
  • Strong assessment, communication, and care coordination skills
  • Valid driver's license and auto liability insurance
