Job Summary

Work in the Transitional Care Unit to provide direct care of adult and geriatric patients undergoing prolonged general medical or post‑operative recovery. The nurse works collaboratively within an interdisciplinary team to ensure smooth transitions to home, providing immediate care and education for patients and families.

Responsible for managing the care of adult or geriatric patients with general medical conditions or surgical procedures. Core duties include:

• Assess patient condition
• Administer medications
• Change dressings
• Monitor vital signs
• Maintain accurate documentation in the medical record
• Provide support and education to patients and families

• Document care in the medical record
• Provide patient and family support and education

• Education: Graduate of an approved practical nursing program (accredited).
• Experience: Previous nursing experience preferred.
• Skills: Excellent verbal and written communication, effective knowledge of scope of practice, strong time‑management and critical‑thinking/problem‑solving skills, flexibility to work in a multitasking environment.
• Licensure/Certification: Must hold a valid LPN license in Missouri and BLS certification obtained within 90 days of graduation; must pass NCLEX‑PN within 90 days of graduation.
• IV certification active in the state of Missouri.
