Position: Travel Licensed Practical Nurse Long-Term Care Job

Overview

Licensed Practical Nurse, Long-Term Care, Jackson, New Jersey, beginning 06/01/2026 for a 12-week travel assignment in a skilled nursing setting. Weekly pay ranges from $1,416 to $1,516, with guaranteed 40 hours per week. Shifts include a combination of 12-hour night shifts (7:00 pm–7:00 am) and 8-hour day/evening shifts (3:00 pm–11:00 pm). This assignment is travel-temporary and supported by Healthforce Healthcare LLC.

Responsibilities
  • Provide direct patient care under supervision, including medication administration per orders and facility policy.
  • Perform resident assessments and document findings in PCC electronic medical record with accuracy.
  • Monitor vital signs and resident status; implement safety and falls-prevention protocols.
  • Administer prescribed medications and treatments; report adverse events and respond to changes in condition.
  • Assist with activities of daily living and routine nursing tasks, including personal care and mobility support.
  • Collaborate with interdisciplinary team (RNs, CNAs, therapists) to coordinate care and ensure smooth shift handoffs.
  • Maintain infection control standards, proper hand hygiene, and equipment sanitation.
Requirements
  • Current, active LPN license (New Jersey preferred or as required by regulatory standards)
  • Proficiency in PCC electronic medical record system
  • Flexibility to work across all facility units and to cover both 12-hour night shifts (7:00 pm–7:00 am) and 8-hour shifts (3:00 pm–11:00 pm)
  • Experience in long-term care or skilled nursing settings preferred
  • Ability to work in a team environment and follow established care plans
