Signify Health, part of CVS Health®, is seeking a Full‑time Nurse Practitioner to provide In‑Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests (e.g., diabetic eye exams, spirometry, etc.).

• Deliver in‑home health evaluations using an iPad and intuitive clinical workflow.
• Bring compassion and professional expertise to each visit, building trusted relationships with patients.
• Conduct virtual visits as needed.
• Provide Diagnostic and Preventive Services products (spirometry, DEE, etc.) as indicated.
• Diagnose health issues; manage patient care while not prescribing medicine, ordering lab tests, or altering treatment regimens.
• Travel locally and within licensed states as required.

As an in‑home clinician, you will work in private residences of health‑plan members. Workplaces may include household pets, secondhand smoke, and other potential airborne allergens or irritants. Candidates must be able to comfortably and safely perform duties in such environments.

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).
• Clinical experience in primary or preventive care (preferred).

Full time, anticipated 40 hours per week.

The typical pay range for this role is $78,331.00 – $, based on experience, education, geography and other factors.
