In-Home Traveler Nurse Practitioner or Physician Assistant - Missouri

Signify Health, part of CVS Health®, is seeking a Full‑Time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations. The role is predominantly travel‑based—up to 100% of the time—within a defined coverage area. Multiple state licenses may be required, and overnight hotel stays, car rentals, or flights may be necessary depending on the coverage area.

• Help visit millions of people nationwide with the goal of connecting them to the right care for their unique needs.
• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
• Maintain flexibility to travel locally and across licensed states.
• Conduct virtual visits as needed.
• Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products such as spirometry, diabetic eye exams, bone density screening, and more.

As an in‑home clinician, your primary workplace is the private residence of health‑plan members. You must be comfortable and safe performing duties in environments that may contain household pets, second‑hand smoke, or other potential airborne allergens or irritants.

The typical pay range for this role is $78,331.00 – $. The actual base salary offer will depend on experience, education, geography, and other relevant factors.
