Position Summary

This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and with standards of professional nursing practice, utilizing the framework for professional nursing practice and development. The position is accountable for the quality of nursing services delivered by self or others under the RN’s direction. The RN uses specialized knowledge, judgment, and nursing skills to assess data and plan, provide, and evaluate care appropriate to the patient’s physical and developmental/age needs.

Functions

Assess patient physical, psychological, social, spiritual, educational, developmental, cultural, and discharge planning needs. Review patient history and physical with patient/family and ensure completion within appropriate timeframe. Review available information obtained by other health care team members. Review diagnostics and laboratory data and report abnormal results to the physician(s) and other appropriate caregivers. Complete assessments and reassessments according to patient need and as outlined in policy.

Formulate a plan of care, including the discharge plan, utilizing assessment data and patient, family and health team input. Initiate a plan of care based on patient‑specific needs, assessment data, and the medical plan of care. Set realistic, measurable goals for the patient in collaboration with the patient/family. Consider the patient’s physical, cultural, psychosocial, spiritual, age‑specific and educational needs. Plan care in collaboration with multidisciplinary team members.

Review and revise the plan to reflect changing patient needs.

Implement the plan of care through direct patient care, coordination, delegation and supervision of the health care team. Provide care based on physician orders and the nursing plan, in compliance with policies and procedures, standards of care, and regulatory agency requirements. Delegate appropriately and provide nursing supervision to other licensed nurses and personnel. Promote continuity of care by accurately communicating patient status to other caregivers.

Evaluate the patient’s and family’s response to care and teaching, and revise the plan of care as needed. Evaluate patient progress toward goals and expected outcomes in collaboration with other health care team members. Evaluate patient response and effectiveness of patient teaching.

Document assessment, planning, implementation, and evaluation in the patient record. Documentation is legible, timely and aligns with policy. It reflects objective/subjective data, nursing interventions and patient response to treatment. Note physician orders accurately and timely.

Provide care based on the best evidence available and may participate in research activities within clinical practice. Participate in unit or facility shared leadership. Interact and participate in education, role development, and orientation of facility personnel, patients, students, families, and visitors. Promote/support growth of others through precepting and mentoring when appropriate.

Contribute to society through activities that lead to excellent patient outcomes through timely, effective, efficient, equitable, and safe care. Actively participate in the improvement of national nursing quality indicators and outcomes, including participation in professional organizations.

In some roles, supervise staff and workflow of the department.

Interact with all levels of staff in a variety of departments, physicians, patients, families, and external contacts such as community providers and agencies concerning the patient’s health care. Provide spiritual support by working with clergy. Synthesize and prioritize data from multiple sources to support the patient and family’s responses to health status changes.

Minimum Qualifications

Current, valid RN license in the state of practice, temporary/interim RN license, or compact RN licensure for the state of practice.
