Nurse Practitioner - Healthcare at Home

Role Overview

Seeking a Nurse Practitioner to provide in-home care without nights, weekends, or holidays, primarily in Greene and Christian Counties.

• Conduct comprehensive in-home assessments, including medical history, medication reconciliation, vital signs, and physical exams.
• Manage acute medical situations and chronic condition exacerbations through testing, diagnostics, and interventions.
• Oversee short-term sub-acute rehabilitation episodes.
• Ensure accurate documentation, including ICD-10 coding.
• Collaborate with interdisciplinary care teams and engage in advanced care planning discussions.
• Educate and counsel patients and caregivers.

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Unrestricted licensure in Missouri and a valid driver’s license with reliable transportation.
• Ability to navigate diverse environments.
• Preferred: 1+ years of clinical experience, familiarity with geriatrics, chronic illness, and electronic medical records.

Salary ranges from $109,500 to $164,000, plus bonuses, with comprehensive benefits, incentive programs, and retirement options. The role offers a supportive environment focused on growth and making a positive impact on patient health.
