As an integral member of our nursing team, we are seeking a highly skilled and compassionate Registered Nurse (RN) to serve as our MDS Coordinator
. In this critical role,
MDS Coordinator will work closely with our nursing staff, healthcare providers, and residents to ensure the highest quality of care and support.

• Coordinate the maintenance and completion of Minimum Data Set (MDS) assessments, care plans, and documentation for our residents
• Collaborate with nursing staff, healthcare providers, and residents to identify and address changes in resident conditions and needs
• Develop and implement individualized care plans that meet the unique needs of our residents
• Ensure accurate and timely completion of MDS assessments and care plans
• Maintain accurate and up-to-date records and reports
• Participate in quality improvement initiatives to enhance the overall quality of care and services

• Current Registered Nurse (RN) licensure in the state of Missouri
• Experience in a long-term care or senior living setting preferred
• Strong clinical knowledge and skills in geriatric care
• Excellent communication, interpersonal, and organizational skills
• Ability to work effectively in a fast-paced environment and prioritize tasks and responsibilities
