The Care Manager collaborates with an interdisciplinary team to assess, plan, advocate, and coordinate patient care from admission to discharge, ensuring safe post-hospitalization transitions.

Responsibilities include implementing effective transition planning, overseeing length of stay, and maintaining high-quality patient experiences, safety, and care standards using performance metrics and best practices.

• Graduate of an accredited School of Nursing;
  Bachelor's Degree in Nursing required
• 2-3 years of experience in an acute care hospital setting preferred
• Current nursing license in the applicable state
• Basic Life Support (BLS/CPR) certification required at hire or within 90 days
• Case Management certification preferred

• Strong assessment and care coordination abilities
• Excellent advocacy and communication skills
• Ability to work collaboratively within a team
