Job Location: Green County
The salary indicated represents a base pay rate. If the individual selected or the position is eligible for a pay differential (e.g., shift, security, or years of service), it will be added to the total compensation in your paycheck. A pay differential does not raise your base pay.
Responsibilities- Assist clients in navigating services and resources available through the Department of Health and Senior Services.
- Conduct assessments to determine individual needs and eligibility for programs.
- Provide support and counseling to clients, helping them to set and achieve personal goals.
- Collaborate with community organizations to enhance service delivery and client support.
- Maintain accurate records and documentation of client interactions and case management activities.
- Participate in training and professional development opportunities to enhance skills and knowledge in social services.
- Bachelor’s degree in Social Work, Human Services, or a related field.
- Experience in a related field may substitute the degree requirement.
- Strong communication and interpersonal skills to effectively engage with clients and stakeholders.
- Ability to empathize with clients and understand their unique needs.
- Organizational skills to manage case documentation and follow‑up activities.
- Proficiency in using computer systems and software for case management.
- Experience working in public health or social services environments.
- Knowledge of local community resources and programs.
- Certifications in counseling or case management.
- Strong team collaboration skills and a positive attitude.
