What You’ll Do:
  • Manage a caseload involving a wide range of disabilities in a designated county area.
  • Provide housing stability case management services for homeless and at‑risk individuals and families.
  • Provide extensive case management, including minimal bi‑weekly contact with each client in combination with phone and face‑to‑face visits in varied settings, including the client’s home.
  • Complete written and data‑based documentation (i.e., internal documentation, case notes, housing plan, etc.).
  • Interview clients, their families and other responsible individuals; collect basic data and obtain appropriate additional information from other agencies; participate in interdisciplinary teams to review each case.
  • Document activities in daily case notes, complete assessments, and other documentation concerning the client’s case (document no less than 55 service hours).
  • Develop a person‑centered plan (individual support plan) for each client in collaboration with other staff.
  • Attend interdisciplinary team meetings to discuss client cases in relation to funding eligibility, support plans, progress, and possible changes.
  • Prepare purchase‑of‑service authorizations and arrange for clients to access services.
  • Visit provider service agencies, attend meetings, and confer with other staff in the program to become more informed concerning provider resources for persons with intellectual disabilities/ developmental disabilities.

· Perform other duties as assigned.

Requirements All you need for success:

Minimum Qualifications

Required:
  • Bachelor’s degree in social work (BSW), Human Services, or equivalent human services degree; OR
  • This position requires a valid driver’s license, a reliable vehicle, and proof of insurance.
Preferred:
  • Experience in case management and homeless/at‑risk of homelessness populations.
  • Knowledge of social services and referral sources for families.
  • Motivational interviewing skills.
