• Provide direct dietary assessment of patient.
• Provide education to patient and caregivers regarding diet management.
• Appropriately report changes and document pertinent information and care rendered to patient to ensure continuity of care.
• Provide counseling and education to IDG, nursing staff and physician, as appropriate.

• Bachelor’s degree in nutrition or related subjects, preferred.
• Current State Driver’s License.
• Two years of experience as a Dietitian in a clinical care setting, hospice or home health preferred.
• Ability to establish a plan of care to meet the dietary needs of the patient.
