Social Service Assistant

Overview

Aventura at Oakwood Village is now hiring a Social Services Assistant (Full-time). Join our compassionate team and make a meaningful difference in the lives of our residents every day. The Social Services Assistant supports the social, emotional, and psychosocial well-being of residents in our skilled nursing facility.

• Assist with admissions, transfers, and discharge planning processes
• Support residents and families with adjustment to facility placement
• Help coordinate appointments, transportation, and community resources
• Maintain accurate social service documentation in compliance with state and federal regulations
• Participate in care plan meetings and interdisciplinary team collaboration
• Address resident concerns, grievances, and psychosocial needs
• Assist with Medicaid applications, insurance paperwork, and resource referrals as needed
• Provide emotional support and advocacy for residents and families
• Ensure resident rights are upheld at all times

• Prior experience with social services is required
• Previous experience in skilled nursing, long-term care, or similar setting preferred
• Bachelor’s degree in Social Work, Human Services, Psychology, or related field preferred (or equivalent experience)
• Strong communication, interpersonal, and organizational skills
• Compassionate, patient-centered approach
• Knowledge of long-term care regulations is a plus
• Ability to maintain confidentiality and professionalism
