PRN Physical Therapist; PT

• Flexible PRN scheduling

• Skilled nursing and long-term care setting

• Utilize Net Health and PCC documentation systems

• Opportunity to build consistent PRN caseloads

• Evaluate residents and develop individualized treatment plans

• Provide skilled physical therapy services focused on mobility, strength, balance, and functional independence

• Accurately document evaluations, treatments, and progress in Net Health and PCC

• Collaborate with nursing, therapy, and interdisciplinary teams

• Maintain compliance with facility and regulatory standards

• Current Ohio PT license

• SNF/LTC experience preferred but not required

• Experience with Net Health and PCC preferred

• Strong communication and clinical skills

• Compassionate, dependable, and team-oriented mindset
