About Us
We are a resident-centered Skilled Nursing Facility in Springfield, Ohio providing compassionate long-term care, short-term rehab, and memory care services. We are looking for a dedicated Social Services Director to join our interdisciplinary care team.
The Social Services professional ensures residents and families receive quality psychosocial support, transition planning, and advocacy services. This role helps promote dignity, emotional wellness, and resident rights throughout the continuum of care.
Key ResponsibilitiesConduct psychosocial assessments for new admissions
Participate in care planning meetings and IDT collaboration
Assist residents and families with discharge planning / community referrals
Support residents through adjustment to facility living
Document resident progress in accordance with state and federal regulations
Educate residents and families about available community resources
Uphold resident rights, dignity, and satisfaction
Experience in long-term care, skilled nursing, or healthcare setting preferred
Strong communication and care coordination skills
Knowledge of Medicaid/Medicare guidelines preferred
