Home Health Occupational Therapist

Become a part of our caring community. As a therapist at Center Well Home Health, reporting to the Home Health Branch Administration, you will play a vital role in helping patients regain strength, mobility, and independence—all from the comfort of their homes.

Responsibilities
  • Assess and screen patients’ daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
  • Administer and interpret diagnostic and prognostic tests of function to patients in their homes or other settings to assist the physician in evaluating the patient’s level of function.
  • Confer with the patient’s physician and other health care team members and participate in the development/revision of the Plan of Care Treatment.
  • Provide therapeutic treatment and instruction to patients in accordance with physician orders to improve or restore strength, coordination, range‑of‑motion, and function or to teach compensation measures.
  • Review and expand the retraining programs in a manner consistent with each patient’s behavioral goals and within the guidelines of the rehabilitation program as a whole.
  • Consult with other vocational team members, as appropriate, to develop and implement vocationally oriented plans consistent with patients’ needs and capabilities.
  • Report patient status and progress to the physician, rehabilitation staff, Clinical Manager, patient, and family members.
  • Participate in care coordination and discharge planning activities and act as a resource to other health care personnel in meeting patients’ needs.
  • Design community reintegration activities to assist the client in physical reconditioning or psychological adjustment and coordinate the plan with members of the interdisciplinary team.
  • Recommend and/or design special adaptive equipment for clients to improve residential or working environments or enhance participation in the rehabilitation program and/or community.
  • Design and train staff and family members to carry out the retraining program, including dressing, feeding, grooming, and hygiene skills; participate in team and family educational meetings.
  • Supervise, instruct, and evaluate Occupational Therapy Assistant and Home Health Aide performance in implementing occupational therapy services.
  • Accurately and thoroughly document patient care observations, interventions, and evaluation on the day services are rendered.
  • Ensure that interim (verbal) orders received from the physician are accurately documented and implemented.
  • Submit evaluation, treatment plans, and discharge summary to the supervisor and care management staff.

Required Qualifications
  • Current and unrestricted OT licensure
  • Current CPR certification
  • Valid driver’s license, auto insurance, and reliable transportation
Preferred Qualifications
  • Minimum six months of occupational therapy experience
  • Home Health experience
Pay

Pay Per Visit/Unit Rate: $54.00 - $76.00

Additional Information

TB Statement:
This role is considered patient facing and is part of Humana’s Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Driving Statement:
This role is part of Humana’s driver safety program and therefore requires an individual to have a valid state driver’s license and to maintain personal vehicle liability insurance. The individual must carry vehicle insurance in accordance with their residing state’s minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, whichever is higher.
