Job Description:
The Physical Therapist will provide home health care services to patients with various conditions including CHF, orthopedic issues, and pneumonia. This role requires a high level of professionalism and clinical expertise.
Responsibilities:
- Provide evaluation, recertifications, and routine visits as required.
- Treat an average of 6.5 patients per day across specified zip codes.
- Collaborate with the clinical manager or educator for patient care coordination.
Requirements:
- Minimum 1 year of experience, preference for candidates who are not new graduates.
- BLS certification required.
- Ability to work under a rotating schedule including weekends and holidays.
