Forest Glen, a skilled nursing and rehabilitation facility in Springfield, Ohio, is seeking compassionate and dedicated Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to join our team. As a nurse at Forest Glen, you will play a vital role in delivering high-quality, resident-centered care while collaborating with an interdisciplinary team to ensure the best outcomes for our residents.

• Provide direct nursing care in accordance with individualized care plans
• Administer medications and treatments safely and accurately
• Monitor residents’ conditions and communicate changes to physicians and care teams
• Maintain accurate and timely documentation
• Supervise and support nursing assistants as needed
• Promote a safe, comfortable, and respectful environment for residents
• Ensure compliance with Ohio nursing standards and facility policies

For LPN:

• Current, valid LPN license in the State of Ohio
• Graduate of an accredited practical nursing program

For RN:

• Current, valid RN license in the State of Ohio
• Graduate of an accredited nursing program

For Both:

• Strong clinical, organizational, and communication skills
• Compassionate, patient-centered approach to care
• Ability to work effectively in a team environment
• Prior long-term care or skilled nursing experience preferred, but not required
