The Social Services Assistant works closely with the Social Services Director and interdisciplinary team to help residents and families navigate the transition to long-term care, access resources, and maintain the highest quality of life possible. This role provides support with care planning, resident advocacy, discharge planning, and family communication.
Qualifications:- Prior experience with social services is required
- Previous experience in Skilled nursing, long-term care, or similar setting preferred
- Bachelor’s degree in Social Work, Human Services, Psychology, or related field preferred (or equivalent experience)
- Strong communication, interpersonal, and organizational skills
- Compassionate, patient‑centered approach
- Knowledge of long‑term care regulations is a plus
- Ability to maintain confidentiality and professionalism
- Assist with admissions, transfers, and discharge planning processes
- Support residents and families with adjustment to facility placement
- Help coordinate appointments, transportation, and community resources
- Maintain accurate social service documentation in compliance with state and federal regulations
- Participate in care plan meetings and interdisciplinary team collaboration
- Address resident concerns, grievances, and psychosocial needs
- Assist with Medicaid applications, insurance paperwork, and resource referrals as needed
- Provide emotional support and advocacy for residents and families
- Ensure resident rights are upheld at all times
