Villa Springfield is seeking a compassionate and dedicated Social Services Director to join our team. This full-time position plays a vital role in supporting the emotional, social, and psychosocial well‑being of our residents and their families.

Position Summary

The Social Services Director is responsible for coordinating and delivering social services that enhance residents’ quality of life. This role involves working closely with residents, families, and interdisciplinary team members to assess needs, provide support, and ensure residents receive appropriate services and resources.

Responsibilities
  • Assess residents’ psychosocial needs and develop appropriate care plans

  • Provide counseling and support to residents and families

  • Coordinate discharge planning and community resource referrals

  • Advocate for residents’ rights and well‑being

  • Collaborate with nursing staff, administration, and interdisciplinary teams

  • Assist with care conferences and family communication

  • Maintain accurate documentation and compliance with regulatory requirements

Qualifications
  • Licensed Social Worker (LSW) preferred but not required

  • Candidates with relevant social services or healthcare experience will be considered

  • Strong interpersonal, communication, and organizational skills
