Forest Glen is looking for a compassionate, dedicated, and organized Social Services Director to join our team! If you are passionate about advocating for residents, supporting families, and helping create a positive quality of life for those we serve, we would love to hear from you.

• Serve as an advocate for residents and their rights, needs, and preferences

• Develop meaningful relationships with residents and their families

• Assist residents with psychosocial needs, adjustment to the facility, and transition planning

• Coordinate and participate in care plan meetings and family conferences

• Maintain accurate and timely social services documentation

• Assist with admissions, discharge planning, and transitions of care

• Address resident and family concerns and help facilitate appropriate resolutions

• Connect residents and families with community resources and services

• Collaborate with nursing, therapy, activities, dietary, and other departments to provide person-centered care

• Promote a welcoming, supportive, and respectful environment for residents

• Experience in social services, preferably in a skilled nursing or long-term care setting

• Bachelor's degree in Social Work, Psychology, Sociology, or a related field preferred

• Strong communication and interpersonal skills

• Compassionate and resident-focused approach

• Excellent organization and follow-through

• Ability to work effectively with residents, families, and an interdisciplinary team

• Knowledge of long-term care regulations and resident rights is a plus
