Peace Health is seeking a Clinical Social Worker Lead for a Full Time, 1.00 FTE, Day position.

The salary range for this job opening at Peace Health is $48.52 – $72.78. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc.

In the Behavioral Health Unit, the Clinical Social Worker Lead provides both clinical expertise and programmatic leadership to support high‑quality patient care and effective team functioning. This role oversees and guides comprehensive biopsychosocial assessments, ensures consistent, evidence‑based treatment planning, and supports complex care coordination and discharge processes for patients with acute behavioral health needs. The Lead Clinical Social Worker serves as a clinical resource and mentor to social work staff, promotes trauma‑informed and patient‑centered practices, and collaborates closely with interdisciplinary leadership to address system barriers, improve workflows, and maintain compliance with regulatory and best‑practice standards.

In addition to direct clinical involvement as needed, this role plays a key part in strengthening transitions of care, fostering strong community partnerships, and supporting safe, timely movement to the next appropriate level of care.

Job Summary

Responsible for coordinating the day-to-day functions for assigned area. Assists with ensuring that departmental goals are achieved, timelines met and service excellence provided in all aspects of work. Subject matter expert. The Clinical Social Worker coordinates the development and implementation of a patient care/treatment plan through use of diagnosis and clinical assessment of psychiatric, chemical dependency and psychosocial needs as they relate to medical and behavioral care needs.

Provides disposition and discharge planning, risk assessment, case management, and coordination of services to provide continuity of care.

Details of the position:

  • Assigns work; plans and manages priorities; provides technical assistance; oversees staff schedules; monitors quality of work; monitors team projects; mentors staff; provides constructive feedback; oversees staff training; ensures quality improvement; provides leadership feedback on staff performance; and assists with goal setting for team.
  • The Clinical Social Worker completes appropriate intake documentation, provides ongoing assessment, develops treatment plan in collaboration with the multi-disciplinary treatment team. The Clinical Social Worker also manages multiple complicated cases, maintains current knowledge of ever-changing community resources, monitors patient needs and progress, and develops aftercare plans. Maintains documentation on patient throughout hospitalization.
  • Provides brief therapeutic interventions with patients and/or families, as well as community staffings, in coordination with the physician and treatment team. Works as an integral member of the treatment team in the provision of treatment and aftercare planning.
  • Ensures compliance with privacy and confidentiality laws. Interfaces regularly with legal entities to address treatment needs.
  • Assists with monitoring and evaluation of services provided for department quality improvement plan.
  • Maintains current understanding of issues related to culture, ethnicity, gender identity, age, sexual orientation, religion and utilizes appropriate communication skills.
  • Provides information, education and direction in response to community requests.
  • Regularly accesses pharmaceutical and community resources to provide patients with needed psychiatric medications.
  • Provides instruction, training and supervision for graduate level social work students. Also provides clinical supervision toward licensure for CSWA's. Must maintain current licensure through continuing education. Must uphold all ethics, rules and regulations required by the Clinical Board.
