What You'll Do

You'll be an important part of helping patients move from hospital care to the next stage of recovery.

In this role, you will:

  • Help arrange home health services and medical equipment needed after discharge.
  • Coordinate follow-up appointments with primary care providers and specialists.
  • Assist patients who do not currently have a primary care provider in establishing care.
  • Support arrangements for outpatient therapy and other post-hospital services.
  • Provide discharge planning support in partnership with RN Care Managers and Social Workers.
  • Document care coordination activities accurately in the electronic medical record.
  • Deliver Medicare Important Message letters and verify patient demographic information.
  • Communicate with clinics, providers, insurers, and community partners to support patient care transitions.
  • Welcome visitors, answer phone calls, and connect people to the appropriate team members.
  • Support department operations through scheduling, reporting, data entry, and other administrative tasks.
  • Help ensure patients, families, and care teams receive timely information and support.

What You'll Bring

Required

  • High School Diploma, GED, or equivalent.
  • Three years of administrative experience in a professional office setting.
  • Strong communication and customer service skills.
  • Excellent organizational skills and attention to detail.
  • Ability to manage multiple priorities in a fast-paced healthcare environment.
  • Proficiency with Microsoft Office applications, including Word, Excel, Outlook, and PowerPoint.

Preferred

  • Experience in a medical office, healthcare setting, or hospital environment.
  • Administrative assistant or business support training.
  • Familiarity with electronic medical records and healthcare terminology.
