You'll be an important part of helping patients move from hospital care to the next stage of recovery.
In this role, you will:- Help arrange home health services and medical equipment needed after discharge.
- Coordinate follow-up appointments with primary care providers and specialists.
- Assist patients who do not currently have a primary care provider in establishing care.
- Support arrangements for outpatient therapy and other post-hospital services.
- Provide discharge planning support in partnership with RN Care Managers and Social Workers.
- Document care coordination activities accurately in the electronic medical record.
- Deliver Medicare Important Message letters and verify patient demographic information.
- Communicate with clinics, providers, insurers, and community partners to support patient care transitions.
- Welcome visitors, answer phone calls, and connect people to the appropriate team members.
- Support department operations through scheduling, reporting, data entry, and other administrative tasks.
- Help ensure patients, families, and care teams receive timely information and support.
- High School Diploma, GED, or equivalent.
- Three years of administrative experience in a professional office setting.
- Strong communication and customer service skills.
- Excellent organizational skills and attention to detail.
