Healthcare Provider - House Calls

Role Overview :

As a House Calls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will perform in-home health assessments for Medicare Advantage and other plan members, focusing on preventive care and closing care gaps.

• Conduct comprehensive assessments including medical history, medication reconciliation, vital signs, and physical exams.
• Identify and document diagnoses for care management and collaborate with primary care providers.
• Recognize urgent situations and intervene appropriately.
• Educate members on health conditions, medications, and promote adherence.
• Address social determinants of health and provide referrals as needed.

• Active, unrestricted licensure in practice state; for NPs, applicable certification (ANCC or AANP); for PAs, NCCPA certification and licensure.
• Reliable transportation and ability to navigate diverse home environments.
• Preferably, 1+ years of clinical experience in family, geriatric, or home health care.
• Proficiency with electronic medical records and strong communication skills with geriatric/Medicare populations.

Role offers flexible scheduling (8am-8pm, 7 days/week), with no on-call, weekends, or holidays.

Compensation ranges from $109,500 to $164,000, plus benefits and incentives.
