The Wraparound Facilitator provides Wraparound Facilitation for eligible youth and families; facilitates Wraparound Teams to develop Plans of Care; works collaboratively with community partners to arrange needed supports; coordinates and collaborates with system of care/community partners.

Duties and Responsibilities
  • Maintains a caseload of up to 15 youth. Maintains records of Wraparound Team meetings, evaluations, individual services and support plans, referrals, follow-up reports, and case notes.
  • Maintains accurate and detailed administrative records including time tracking, mileage, and other critical records as required.
  • Facilitates Wraparound Team Meetings, and helps develop, and implement individual Plans of Care with the team.
  • Works with CCO Care Managers to plan and coordinate the activities of wraparound service with the mental health providers and system of care.
  • Works with other youth serving agencies to implement coordinated and effective service provision to youth and their families.
  • Works with families to build informal networks and social supports including engaging with child and family advocacy programs.
  • Advocates as appropriate on behalf of families for needed supports from system of care community partners.
  • Provides information on and referrals to specialty services; arranges for a variety of goods and services to meet the needs of a youth or family as identified by the Wraparound Team including but not limited to respite care; medical; social or recreational services; etc.
  • Communicates new information/resources to YFS staff.
  • Maintains and increases knowledge through reading, attending a variety of training programs and conferences, and other resources.
  • Adheres to Oregon Administrative Rules pertaining to Wraparound Care Coordination as required by the State of Oregon (OAR ).
  • Collaborates with other YFS staff to address issues of common concern.
  • Maintains a flexible work schedule, and travels to families’ homes and community locations when needed.
  • Communicates and maintains schedule using agency’s communication and calendar applications.
  • Participate in workspace norms and duties including maintenance and upkeep of agency spaces.
  • Provides case studies or progress reports as directed.
  • Participates in fundraising, development, and outreach activities.
  • Performs other projects/duties as assigned.

  • The ability to use standard office equipment, mobile technology, computers, and computer software such as Electronic Health Record and the Microsoft 365 Suite. Ability to learn other computer software programs as required by assigned task.
Minimum Position Qualifications
  • A bachelor’s degree or combination of three years’ experience in a related field.
